Medical documentation will remain on file for future reference as needed. To ensure medical privacy, no specific medical diagnosis, treatment, or prognosis information is necessary to be on the medical clearance form.

NOTE: PLEASE, PLEASE DO NOT RAISE OR LOWER THE BLINDS IN EITHER THE BILLIARD OR FITNESS ROOMS!! Doing so will damage the blinds and make them inoperable. The blinds are designed to be opened and closed only. This is done by using the wand on the right side of each blind.

STREET SHOES AND FITNESS ROOM
This is the time of year when we ask our residents to not wear their street shoes into the fitness room while using the equipment. The sand and salt that is picked up when walking outside is not a healthy diet for the equipment. This can cause serious problems for the motorized parts of the cardio-equipment thus leading to repairs and/or costly replacement. Please bring a change of shoes to use in the fitness room. Thank you. [Source: Overlook Overview January 24, 2008]
